PKDL usually occurs within the first six months after treatment. It is more common in HIV co-infected patients. It may be a sign of immune reconstitution. The treatment depends on the severity according to the clinical grade. While patients with grade 1 and 2a may have spontaneous recovery, grade 2b and 3 need drug treatment. PKDL may act as reservoir and play a role in transmission of disease.  

Such patients remain with the disease and with low immunity. They are predisposed for other opportunistic infections and their survival is compromised. As they always harbor high parasitemia, they can act as reservoirs in the disease transmission in the community.
